New Spa Reward Program goes into effect June 1, 2008. The old existing program will end and a new reward program will take effect. We encourage all of you to fill up your present cards and get them redeemed. Also a New Retail Reward Program is in the making to start here soon.
Upon the 14th service, turn this card in and a Spa Reward card will be mailed out to you within 30 days of receipt of the complete card.
Retail does not qualify for this program.
Spa Reward card will be the average of all services paid for and recorded on this card (equal to 1/14th of the total of all services paid for and recorded on this card).
Spa Reward card will NOT be issued without completed card.
Any coupons, discounts or special offers will be deducted from amount of service before it is recorded on this card.
Customer is responsible for maintaining this card and ensuring that the Front Desk Staff fills out the card at the time of purchase.
Replacement Card will only reflect the last 6 months of sales history and will only be provided once in 12 months.
Cannot be redeemed for cash.
Offer may be changed or cancelled at any time.
The Spa Reward Card must be present to be redeemed (no exceptions). This is a reward promotion not a gift card.
